Chiropractor
Sevak Khodabakhshian, D.C.
Dr. Sevak Khodabakhshian (known to many as Dr. K) is the Clinic Director and founder of Omega Rehab & Sport. A top-honors graduate of Palmer College of Chiropractic West, he has been proudly serving the Conejo Valley community since 2003.
Dr. K specializes in the diagnosis and treatment of complex spine and nerve-related conditions, integrating advanced chiropractic care with injury rehabilitation to deliver comprehensive, evidence-based solutions for recovery and long-term strength. Outside the clinic, he’s an avid surfer who brings the same focus, balance, and respect for movement found in the ocean to his work helping patients move beyond pain and live active, fulfilling lives.
Chiropractor
Zack Fulks, D.C.
A Bay Area native, Dr. Zack Fulks discovered his passion for chiropractic care through his own experience as a football player recovering from injuries with the help of his family chiropractor.
After earning his bachelor’s degree in Pre-Physical Therapy from San Diego State University, Dr. Fulks graduated Valedictorian from Palmer College of Chiropractic West in 2022.
At Omega Rehab & Sport, Dr. Zack focuses on restoring proper joint motion, improving spinal stability, and optimizing movement patterns to help patients recover efficiently and prevent future injury. His care emphasizes education, active rehabilitation, and practical strategies patients can use to stay strong and mobile outside the clinic. He is dedicated to building lasting relationships with his patients and helping them move confidently beyond pain to strength.
Director of Physical Therapy
Ethan Izaki, D.P.T.
Ethan has been a Doctor of Physical Therapy since 2003, specializing in outpatient orthopedics. He has worked with patients across all walks of life—from professional athletes to youth in the local sports community and their families—providing individualized, results-driven rehabilitation.
He earned his undergraduate degree in Biology from the University of California, Santa Barbara, and his Doctor of Physical Therapy from the University of Southern California. A certified clinical instructor, Ethan mentors aspiring physical therapists and brings extensive knowledge of post-surgical rehabilitation, supported by years of surgical observation and clinical leadership. He has successfully managed and directed multiple physical therapy clinics.
Ethan believes every patient deserves a customized treatment plan tailored to their unique challenges. With a passion for sports, travel, and helping others return to what they love, he is dedicated to getting you back in the game.
Physical Therapist
Anne O’Sullivan, M.P.T.
For over 21 years, Anne has been helping patients move better, feel stronger, and live with greater confidence. Her extensive experience spans orthopedics, inpatient care, aquatic therapy, geriatric rehabilitation, and Pilates-based movement, giving her a unique ability to adapt care to every individual’s needs.
Anne began her career as a Physical Therapist Assistant before earning her Master’s degree in Physical Therapy from California State University, Northridge. A certified Pilates instructor, she blends precise movement, postural control, and strength training into her rehabilitation programs to promote balanced, lasting recovery.
A cornerstone of the Omega Rehab & Sport team for more than two decades, Anne continues to inspire patients with her deep knowledge, compassionate care, and unwavering dedication to helping each person reach their full physical potential.
Marketing Director
Tyaga Hebel, N.C.P.T., C.E.S.
Tyaga Hebel began her fitness career in 2000 and is certified through NASM as a Personal Trainer and Corrective Exercise Specialist. With a background specializing in neutral spine training, she has helped countless clients restore proper movement, build core strength, and develop lifelong habits for better posture, balance, and functional performance.
Today, Tyaga brings that same passion and understanding of human movement to her role as Director of Marketing and Community Outreach at Omega Rehab & Sport—connecting people, programs, and partnerships that inspire others to move beyond pain to strength.

Stephen Hirst
A Chicago native, Stephen moved to California at the age of 16. He earned his Associate of Science in Engineering from Ventura College in 2014.
Stephen brings a wealth of knowledge from his diverse experience to his role at Omega Rehab & Sport. In his free time, Stephen enjoys reading, cooking, playing board games, watching movies, listening to music, and exploring the great outdoors through walks, runs, and hikes.
Office Manager
Karen Howell
Karen Howell has been Office Manager at Omega Rehab & Sport since 2004. She has more than 30 years experience in medical office management, specializing in the fields of Physical Therapy and Chiropractic care. Karen is a native Texan and moved her family to California in 1994. She loves spending time with family and friends, crafting, and gardening.
Certified Massage Therapist
Nicole Dowell, C.M.T., N.C.P.T.
Nicole is a Georgia native and moved to California in 2006. She graduated from IPSB at Life Energy Institute In 2008 when she began her massage therapy career. Her specialties are Therapeutic Deep Tissue, Sports Massage, and Trigger Point therapy. Nicole brings with her a love for salsa, tango, ballroom and swing as well as rock climbing, backpacking and hiking.
Personal Trainer
Matt Markowitz, NCPT
Born and raised in the Conejo Valley, Matt has called Newbury Park home since 1978. A lifelong athlete, he began martial arts training at age six and earned his second-degree black belt by twelve. With over 40 years of experience in fitness and sports—from surfing and snowboarding to strength training—Matt brings both passion and discipline to every session.
As an NASM-certified personal trainer currently pursuing his Corrective Exercise Specialist certification, Matt combines real-world athletic experience with evidence-based training. After losing a family member to illness, he made a life-changing commitment to health and fitness—a journey that fuels his purpose to help others build lasting strength, balance, and vitality.

Karla Redondo
Karla is a Baja, Mexico native and recently received her AA in Kinesiology at Moorpark College, CA. Her plans are to finish her bachelor’s degree in Kinesiology and pursue a career in Physical Therapy. She is passionate about helping others and has a great love for sports.

Sara Mills
Sara Mills graduated from California Lutheran University with a degree in Exercise Science, emphasizing Health Professions. Originally from Joshua Tree, California, she has a passion for sports, especially basketball, and enjoys hiking and going to the beach.
Sara is currently pursuing her EMT license and plans to continue her path toward medical school, aspiring to make a meaningful impact in healthcare.
Rehabilitation Aide
Emily Ortega
Emily Ortega is a dedicated Rehabilitation Aide at Omega Rehab & Sport. She earned her Bachelor’s degree in Kinesiology with a concentration in Pre-Physical Therapy and a minor in Health Science from California State University, Dominguez Hills.
Passionate about advancing her career in healthcare, Emily is beginning a Radiologic Technology program, with plans to specialize as an X-ray, CT, and MRI technologist. Outside of the office, she enjoys staying active through hiking, running, and exploring new places—especially discovering great local restaurants.
Rehabilitation Aide
Grace Herrera
Grace Herrera is a kinesiology student with a lifelong love of sports and movement. A former soccer player raised in Thousand Oaks, she’s passionate about supporting athletes from the other side of care and plans to pursue a Doctor of Physical Therapy degree with a focus on sports medicine.
When she’s not studying or gaining hands-on experience at Omega Rehab & Sport, Grace enjoys concerts, music festivals, amusement parks, and peaceful mornings at the beach.

Julianna Saenz
Julianna is a Northern California native and grew up in the small town of Bishop. She graduated from California Lutheran University with a BS in Exercise Science and Sports Medicine. She brings with her over 24 years of providing excellent customer service in the health and medical field. She loves the ocean, to be outdoors and is an amateur scenic photographer.

Joanne Ellis
Joanne Ellis is a Newbury Park native who brings years of front desk and patient relations experience to the Omega Rehab & Sport team. With a strong background in medical reception and customer service, she’s skilled in scheduling, patient check-ins, and ensuring every visitor feels welcomed and supported.
Joanne enjoys connecting with people and helping them have a positive experience from their very first call. Outside of work, she loves hiking and spending time outdoors, embracing the natural beauty of the community she calls home.

Michele Mote
Michele is originally from La Crescenta, California, and now calls Thousand Oaks home. Before joining the Omega Rehab & Sport team, she gained extensive experience in the medical field, including patient care, administrative support, and medical billing. Outside of work, Michele enjoys spending time outdoors and nurturing her passion for gardening.
